1. Daily Medication Schedule to Print

2. Make your own Medication Adherence Chart

Use a word processor like MS Word or MS Works to create your own medication planner. Include;

name of drug, times, dosage, your name and date the chart.

A. Date created; and version number

B. Name of drug; Sinemet, Colace, Vioxx...

C. Medication time; also include frequency like prn

Other optional fields are color, shape etc to help with pill identification.
